Due to COVID-19, our office policy has changed to ensure effective infection control amid the ongoing pandemic.
A mask or face covering is REQUIRED while you are in the waiting room.
Upon check-in with the front desk staff please stand behind the glass barrier.
Only patients with appointments that day may enter the office. Other visitors (with the exception of ONE parent/guardian with a minor patient) will be asked to wait outside.
Patients will not be permitted to enter the premises until the time of their appointment. Please do not enter early.
Due to social distancing, patients will be seen on a strict schedule. Late patients will not be seen.
All new patients will be required to complete new patient forms online ahead of their appointment to limit contact with surfaces.
Appointments are in high demand and are filling quickly. Please be prepared with your insurance information and scheduling preferences when you schedule. If you need to make a payment you may do so over the phone. Receipts will be emailed promptly. Online payments will soon be live on our website for contactless payment.
If you have an BCBS HMO, Cigna HMO, DeltaCare USA, or United Concordia HMO, please make sure you have assigned yourself to our office to prevent scheduling delay. If you are not already assigned to our office at the time of scheduling, you will NOT be able to book and will have to wait up to 30 days for transfer.
Please note that we do not accept Adult Medicaid. If you have a DentaQuest plan we recommend contacting your carrier for other providers.
