Information for New Patients

We are currently experiencing higher than usual call volume during business hours. We strongly recommend patients contact us via email at email@example.com for new patient appointments. We are working very hard to respond to everyone promptly. Emails are usually replied to within minutes. Phone contact may take up to 24 hours for a response.
PLEASE NOTE: Due to the high demand for appointments and the temporary suspension of broken appointment fees we will not be able to schedule multiple family members together. We understand that due to the ongoing pandemic, symptomatic patients may have to cancel. By scheduling patients separately, we are able to prevent losing appointments that other patients need. We appreciate your cooperation.
PROVIDING INSURANCE INFORMATION:
Patients that are unable to provide insurance information (when covered) WILL NOT be able to schedule. Insurance information is required for insured patients to book so that eligibility can be determined that the time of scheduling. This allows us to alert patients of any issues with eligibility prior to their appointment.
Please be ready to provide the name of your insurance (specify HMO or PPO), your ID number, and your date of birth. If you have an HMO you will need to contact your carrier to transfer your primary care assignment to our office. Patients will be able to book an appointment once he/she appears on our patient list/captiation report.
If you have had x-rays taken in the last 3 years please have them transferred to our office at firstname.lastname@example.org. If you do not have any current x-rays, we can update them during your examination. Check-up x-rays of the back teeth will be taken for all patients as part of your exam.
- Please be ready with your schedule and multiple appointment options (popular times are currently not available)
- We prefer that adult patients call to schedule their own appointments. This prevents issues that occur in communication leading to missed and canceled appointments.
