About your HMO Plan

We are providers for BCBS HMO, Cigna HMO, Delta Care HMO, and United Concordia HMO. Patients that are already assigned to us and on our roster are immediately eligible to schedule an appointment. Depending on your plan benefits you may have a small copay. For the most up to date information regarding your coverage, please contact your carrier. Our office is provided an estimate (not a guarantee) of your benefits and will make every effort to assist you.

If you are not already on our roster, you will not be eligible for any treatment until you transfer assignment.


If you are switching dentists you will need to request an office transfer with your dental plan. Patients will not be able to book any appointments with a new dentist until the transfer is effect the following month.


Most HMO plans require that a patient see their primary care dentist prior to going to a dental specialist (endodontist, periodontist, oral surgeon, orthodontist, prosthodontist). Patients that do not obtain a referral from their primary care will usually be responsible for out of pocket costs of seeing the specialist.

Treatment Options

HMO plans tend to cover the cost of Standard/Economy dental options. Premium and Cosmetic options are available but are typically not covered under any HMO plan.
