In order to meet our own high expectations for your care, we work diligently to prepare for your visit. This includes making sure we have all the information we need to better assist you.

At the time of scheduling we ask that you have your insurance information ready. This enables us to obtain a complete benefit breakdown and better explain any insurance questions you may have.  Also, if you need any assistance with pre-medication, medical releases, or special concerns, please let us know in advance. Our goal is to make your visit as easy as possible. 



Insured Patients: Please be ready to provide your insurance information at the time of scheduling. This information is used to verify that we are in-network with your insurance and to avoid any unpleasant surprises. Appointments cannot be reserved without valid information.

Self-Pay Patients: We recommend enrolling in a Patient Care Plan. Benefits start immediately and costs $385 for the entire year. This will cover all of your cleanings, exams, x-rays and emergency visits for the entire year. Additional benefits like fillings, gum treatments, crowns, implants and extractions will be discounted by 15%-50%. 



Help us to devote 100% of your appointment time to you. We ask that all patients complete our new patient forms online. We recommend completing the forms 24 hours in advance to allow us to assist you if you have technical issues. problems. Forms are available in English and Spanish for your convenience.



Please arrive on-time for your appointment. Late arrivals cannot be accommodated as they will result in the unfair delay of the next patient's appointment. 

You first visit will include a complete examination, complete digital series of x-rays (if necessary), full health history, and oral cancer screening. If you have any dental issues that need to be addressed, we will provide you with a treatment plan detailing benefits and coverage. Dental work such as fillings, extractions, crowns ect. will not be completed the same day as your initial appointment.

*If you have x-rays taken in the last twelve (12) months, please email them to We strongly recommend you send your own x-rays to us directly. This ensures that we receive your x-rays on time. Without a copy of your x-rays sent in advance or consent to take new ones during your appointment, we will not be able to accept you as a new patient.
