We are excited to meet you!
In order to meet our own high expectations for your care, we work diligently to prepare for your visit. This includes making sure we have all the information we need to better assist you.
At the time of scheduling we ask that you have your insurance information ready. This enables us to obtain a complete benefit breakdown and better explain any insurance questions you may have. Also, if you need any assistance with pre-medication, medical releases, or special concerns, please let us know in advance. Our goal is to make your visit as easy as possible.
Help us to devote 100% of your appointment time to you. We ask that all patients complete our new patient forms online. We recommend completing the forms at the time of booking to allow us to assist you if you have technical issues. Forms are available in English and Spanish for your convenience.
We ask patients to arrive on-time for their appointment. We value our patients time and cannot delay the next patient's appointment in order to accommodate late arrivals.
Please complete your new patient forms in advance of your appointment. This enables our office to see you on time and use your full appointment time for you, not paperwork.
We strongly recommend emailing your own x-rays to ensure we receive them in time. Requesting x-rays be sent on your behalf, usually results in delays, as offices have up to 30 days to send records. Because most dental insurance have limitations restricting how often they will cover x-rays, it is important for patients to send records to avoid any unexpected patient costs. Without a copy of your x-rays sent in advance or consent to take new ones during your appointment, we will not be able to accept you as a new patient.
