Your first visit will include an exam and all necessary x-rays (age permitting). Any dental concerns/conditions will be provided in a detailed treatment plan. This document will include procedure details, insurance benefits, and any applicable patient copays. You will be texted or emailed a copy for your records.
To reduce the surfaces you need to touch, we now require the completion of our Smart Forms. These forms will automatically be sent to you via email or text for completion prior to your appointment.
We are in network with most major dental plans. At the time of scheduling please make sure to call with your plan name and member ID number. Without your insurance information, we cannot estimate coverage or the possible limitations of your plan.
To prevent surprise bills we require that if you plan to use insurance that we have that information on file at the time of scheduling. If your plan is not effective yet and you are a new patient, we will have to schedule once we can confirm eligibility.
If you have complete x-ray series taken in the last 5 years, please email a copy to . Please note that due to HIPAA you must contact your previous dentist on your own behalf.
Although, x-rays records are helpful, if you present with a new issue, we may need to take new x-rays to identify any new dental problems.
